How to Register Your Child

 For Kindergarten Entrants

Students must be five years of age on or before October 1st of each school year in order to be enrolled in Kindergarten.  Each year, we do a Kindergarten “Round Up” beginning in December, asking parents to call our office for a registration packet at 908-996-2941 ext. 517.

In the Spring (usually March or April), we hold an orientation so that any necessary paperwork can be submitted prior to the Kindergartener’s entrance in September. 

Please read the following and access the links below for the appropriate forms.

  • Please read the two-sided document “Preliminary Information: Please read before proceeding”;
  • Complete the two sided “Kingwood Township School District Registration Form”;
  • Complete the appropriate Affidavit and attach required documentation which best matches the pupil’s residency circumstances.  (Domicile, Affidavit Student or Special Circumstances).   If you have any questions regarding these forms, please contact the Main Office;
  • Birth certificate with original seal;
  • Child’s custody papers, if applicable;
  • Kindergarten Health Examination Record (enclosed form to be completed by your child’s physician after January of the year of entrance and needed prior to the start of school);
  • Immunization record (if the portion on above form not completed);
  • A TB Mantoux test if your child is coming from out of state or from another country;
  • Health History Form (see Kindergarten forms below);
  • Release of Records (see Kindergarten forms below);
  • Child Information Form (see Kindergarten forms below);
  • Applicable Use of the Internet Form (see Kindergarten forms below);
  • Permission to Publish Form (see Kindergarten forms below);
  • Request For Alternate Transportation Form, if applicable (see Kindergarten forms below);

Please note that the emergency information asks you to indicate whether your child is covered under a health insurance policy or not.  If you do note have health insurance for your child, we ask your permission to share the emergency information with the NJ FamilyCare to remain in compliance with FERPA requirements and N.J.A.C.6A:32-7.1(g).  Last year, Governor Corzine signed into law P.L. 2008 Chapter 38, which mandates that all children age 18 and under have health insurance.  The Governor has charged all state agencies to engage in outreach strategies to help families meet this requirement.  Thus, we have been requested to ask you for this information.  If you are interested in NJ Family Care, you can visit their website at http://www.njfamilycare.org  

For your information, New Jersey State Law requires the following immunizations for students who are enrolling in kindergarten:

1.    Diphtheria, Tetanus Toxoids and Pertussis Vaccine (DTP/DtaP) – a minimum of four (4) doses with one dose being administered on or after the fourth birthday.  Alternatively, a child with any total of five (5) doses of diphtheria, tetanus toxoid and pertussis vaccine will also be in compliance with this regulation.  If DT (pediatric diphtheria-tetanus toxoid vaccine) is used instead of DPT, a medical exemption note from a physician or certified registered nurse practitioner explaining why pertussis vaccine was not given must be submitted.

2.    Polio Vaccinea minimum of three (3) doses is required, provided that at least one (1) dose is administered on or after the fourth birthday.  Alternatively, a child with any four doses of polio vaccine spaced by a minimum of 28 days is in compliance with this regulation.

3.    Measles, Mumps, and Rubella (MMR) – all children must have one (1) dose of measles, mumps, and rubella vaccine administered either separately or together as MMR on or after their first birthday.  A second dose of measles, preferably MMR, is to be given at least 28 days after the first dose and prior to the start of kindergarten.

4.    Hepatitis B Vaccine children born on or after 1/1/96 are required to have three (3) doses of hepatitis B vaccine to be in compliance.

5.    Varicella (Chicken Pox) – children are required to have one (1) dose on or after their first birthday.

6.    Haemophilus Influenzae B (Hib) – a minimum of two (2) doses of Hib vaccine is needed if between the ages of 2-11 months; a minimum of one (1) dose of Hib vaccine is needed after the first birthday.

PLEASE NOTE:  Between September 1 and December 31 of the school year, any child who is 59 months of age or younger MUST receive an annual flu shot.  You will be asked to supply this information during that time period.  Children who have turned 60 months of age before December 31 of the school year are NOT REQUIRED to get a flu shot, but the Department of Education strongly recommends this practice.

HOW TO REGISTER CHILDREN IN GRADES 1 AND HIGHER

Students must be six years of age on or before October 1 of the school year to be enrolled in Grade 1.  If applicable, all “transfer” students must supply a transfer card listing the student’s NJ Department of Education student identification number.

  • Complete the Permission to Publish and Acceptable Use of Internet Form;
  • Complete the Release of Records;
  • Provide Health Examination Record;
  • Provide Immunization Record;

  • Provide birth certificate with original, raised seal (copy will be made at school);
  • Child’s Custody Papers, if applicable

STUDENTS RETURNING TO KTS EACH YEAR

The parents of students who continue to be enrolled in Kingwood Township School are sent a packet in June asking them to verify or supply the following information:

  • Verify or make corrections to the emergency information on file
  • Update emergency alert information
  • Technology and Acceptable Use of Internet Form
  • Permission to Publish Form

 

In August, parents are asked to come to the school on specific dates to pick up their child’s schedule and other information.  This information may include:
  • Student homeroom/schedule
  • Pick up/drop off procedures
  • Parent/Student calendar handbook
  • School map
  • Supply List
  • Information from PTA and KTEF
  • Other various information
